Workplace Violence in Healthcare Incident Reporting
The Workplace Violence Reporting Toolkit is a comprehensive resource designed to assist organizations in acute care, long-term care and community care settings in establishing, implementing and maintaining effective protocols for reporting workplace violence hazards and incidents.
The prevalence of violence in healthcare, coupled with high rates of underreporting, places many workers at risk of serious physical and psychological harm. This toolkit provides practical guidance to encourage timely, consistent reporting to help employers create violence-free workplaces where workers receive the protection they deserve.
The toolkit also includes the following supporting resources:
- Appendix A – Sample Hazard and Incident Reporting Policy
- Appendix B – Sample Workplace Violence Hazard Report Form
- Appendix C – Sample Workplace Violence Incident and Injury/Illness Report Form
- Appendix D – Sample Workplace Violence Hazard and Incident Log
- Appendix E – Sample Care Recipient Workplace Violence and Responsive Behaviour Journal Log
- Appendix F – Sample Hazard and Incident Reporting Tracking Tool
